ABSTRACT

The use of current modified rules for intubation can result in better outcome with respect to preventing transmission of COVID-19 infection to healthcare providers. We aimed to assess the effect of implementing preventive and managerial protocols related to tracheal intubation of COVID-19 patients on preventing adverse clinical and psychological outcomes in personnel of intensive care units. Methods: This randomized single-blinded clinical trial study was performed on all personnel of ICU in a referral center for COVID-19 patients in Tehran in April 2020. The personnel were assigned into two group as the intervention group including personnel who received the necessary training to perform intubation based on existing standard and modified protocol and the control group that did not participate in the training sessions and continued their routine performance. Along with the clinical assessments, the personnel were assessed for depression and stress symptoms using the short form of Depression Anxiety and Stress Scale (DASS), 14 days after educational session. Results: There was no difference in clinical symptoms, hemodynamic condition, and laboratory findings across the two groups of personnel. The assessment of psychological aspects in both intervention and control group showed no difference in mean depression score as well as the severity of depressive mood, but the mean stress score in the two groups was 7.42±2.00 and 14.92±5.79 respectively indicating a significant difference (p = 0.007). In this regard, stress in any of its severity was not found in the personnel of educationbased intervention group, however more than half of the personnel in control group suffered from mild to moderate stress. Conclusion: The scheduling and conducting training sessions and curricula related to intubation of COVID-19 patients can be very effective in reducing the level of personnel stress and therefore may reserve their occupational performance and minimized the likelihood of exposure to COVID-19. ABSTRACT

Introduction and objective: Undoubtedly, employees face a variety of organizational problems and issues during the service period. Employees' decisions and actions in facing these problems and issues will be based on their knowledge and mental patterns. This study aimed to document and record the cultural and spiritual experience in the face of the COVID-19 crisis. Method: This research was an applied, inductive and exploratory-descriptive case study, which was conducted as a one-time crosssectional survey in a qualitative manner. Data was collected using documentary reviews and questionnaires. The statistical population of this study included managers, experts, and administrators of Baqiyatallah University located in Tehran, Iran. According to the qualitative approach of the research, purposeful sampling was used and data collection was continued until the required data was collected. Data analysis was performed in MaxQDA 2020 and Excel 2019 software. Results: The results showed a total of 282 open source codes. Code frequencies were as follows: 25 codes for event dimension, 54 for issue dimension, 61 for measures and decisions dimension, 71 for output-outcome dimension, 35 for suggestions dimension, 17 for scenario planning and modeling dimension, and 19 for lessons learned dimension. Conclusion: The results of this study can be used as a basis for managers to plan and implement experience documentation in cultural and spiritual areas in the face of the COVID-19 crisis. ABSTRACT

Background and Objectives: The most appropriate approach to control the SARS-CoV-2 epidemic is the widespread adoption of vaccination. Several vaccines against SARS-CoV-2 have been developed and authorized for use in various geographical regions. The aim of this study is to evaluate the efficacy of the vaccination agents presently utilized by healthcare workers (HCWs), and to investigate whether different COVID-19 vaccines would result in the alleviation of symptoms and the severity of clinical presentation. Materials and Methods: This multi-center survey was conducted on 329 vaccinated HCWs who were reinfected with COVID-19 between January 8, 2021 and April 8, 2021, in Tehran, Iran. Results: Overall, 92.1% and 70.8% of the participants had received 2 and 3 cumulative doses of COVID-19 vaccines, respectively. There were no differences between first/second and third-dose vaccines with the severity of SARS-CoV-2 infection. Expectedly, vaccination resulted in a less severe clinical presentation of SARS-CoV-2 infection, as reported by the participants. Conclusion: The results suggest that the efficacy of the vaccination agents presently utilized by HCWs was acceptable with no significant difference in vaccine type. Participants receiving at least two doses of vaccines in this survey exceeded 90%, which is comparably higher than studies conducted in other countries. ABSTRACT

Background and Objective: The Covid-19 pandemic doubled the need for creative interventions to prevent cardiovascular diseases. Therefore, the present study was conducted with the aim of determining the effect of intervention based on family empowerment on preventive lifestyle of cardiovascular disease risk factors in middle-aged women. Materials and Methods: In this randomized clinical trial study, 74 middle-aged women covered by comprehensive health service centers in the south of Tehran were selected based on inclusion criteria and randomly assigned to two intervention and control groups and were investigated. The family-centered empowerment program based on the preventive lifestyle of the risk factors of cardiovascular diseases was conducted for the intervention group during 6 sessions and each session lasted for 30 minutes virtually and through the WhatsApp application. After 8 weeks, the questionnaires were re-completed and the data were collected using demographic information, Food Frequency questionnaire, Baecke and metabolic health questionnaires and descriptive statistics methods (frequency distribution tables - index numerical values calculation) and inferential statistics (chi-square test, independent t, paired t and Fisher) were used and analyzed in SPSS statistical software version 20. Results: The findings of this study showed that the average age in the control group was 44.03 ± 2.35 in the intervention group was 46.65 ± 3.29 and the intervention and control groups were homogenous in terms of demographic variables, eating habits, physical activity, and metabolic health, and there were no significant differences (P<0.001). After the intervention in metabolic health (control of blood sugar, blood pressure, blood cholesterol and smoking), physical activity and eating habits, there was a significant difference between the two groups, which indicates the effectiveness of the intended intervention (P<0.001). Conclusion: Family empowerment is one of the most effective methods to prevent modifiable risk factors in chronic diseases, especially cardiovascular diseases. The covid-19 pandemic created a good opportunity for families to participate in the implementation of the above programs. Programs that focus on the foundation of the family leads positive effects in the direction of educating people, especially older people in the family, in relation to changing behavior, and it can be used as a safe and effective method for cardiovascular disease prevention. ABSTRACT

Background and Object: Nurses play a vital role in dealing with the COVID-19 crisis, considering that the high level of knowledge and ability of nurses in dealing with this crisis accelerates the recovery process of patients and promotes the health level of the community. Therefore, this study aimed to determine the effectiveness of virtual training package for nurses' scientific empowerment against covid-19 crisis. Materials and Methods: The study consisted of descriptive cross-sectional and semi-experimental two phases study population including 256 nurses in spring 2021 in one of the selected hospitals in Tehran. Data collection scales, were demographic questionnaires and four-choice test Researcher made. Virtual training package was prepared with the purpose of nurses understanding of COVID-19, prevention, diagnostic, Treatment and nursing care methods and education during discharge in the form of multimedia educational files. 63 nurses whose scores were lower than the average of the society were divided into two groups of 32 interventions and 31 controls by block random allocation method. One month after the intervention, post-test was performed and the data were analyzed using descriptive-analytical statistics using SPSS software version 26. Results: The scores of nurses' empowerment in the experimental group before the intervention was 7/96(2/96) and after the intervention was 23/96(1/28) and in the control group before the intervention was 8/96(2/58) and after the intervention was9/62(2/47). according to the results, the scores of both groups before the intervention were not significantly different (P=0/169(but after the intervention, there was a significant difference (P<0/001) the score of nurses who used virtual training package had a significant increase. Conclusion: Virtual training is a suitable method for empowering nurses against with COVID-19 crisis. This study was conducted to meet the needs of medical and educational centers to improve nurses› knowledge and capability against COVID-19 crisis. In this study, while determine the level of nurses› scientific empowerment, the effectiveness of virtual training on nurses› empowerment in these conditions was investigated and confirmed. ABSTRACT

The COVID-19 pandemic has had a major effect on our lives. Many of us are facing challenges that can be stressful and overwhelming and cause strong emotions in adults and children. The purpose of this study was to measure the level of anxiety of non-clinical individuals in the Iranian community towards COVID-19 in Tehran. The present study is a descriptive correlational method with 308 individuals participating in the study through an online recall. A researcher-made coronary anxiety questionnaire with 18 questions was used to collect data. The data were analyzed by using Cronbach's alpha internal consistency and Guttman's λ2 method. Confirmatory factor analysis (CFA) using Lisrel-8.8 software was used to evaluate the tool construct validity. To standardize the raw scores, they were converted to standard T scores and percentile rank using Jmetrik-4.1.1 software and were prepared as normative tables. The Guttman's λ2 value for the whole questionnaire was obtained as (λ = 0.922), Cronbach's alpha coefficient for psychological symptoms as (α = 0.879), physical symptoms as (α = 0.861), and for the whole questionnaire as (α = 0.919). The range of scores of the questionnaire factors and total score of COVID-related anxiety severity based on standard T scores was divided into three domains: mild, moderate, and severe. The increase in psychological and physical symptom scores of coronavirus anxiety was significantly associated with physical symptoms, anxiety, depression, and social dysfunction. The fact that anxiety acts as a predisposing and health-threatening variable has confirmed that other researchers should examine its psychometric properties in other populations (especially compared to the clinical population to increase the diagnostic value of this questionnaire) as well as in relation to other psychological, social and medical concepts and variables. © 2022 Copyright 2022 by Koninklijke Brill NV, Leiden, The Netherlands.

ABSTRACT

Background and Objective: Today, the issue of health aspects in urban waste management, especially recycling, has received a lot of attention during Covid-19 around the world. Therefore, the purpose of this research is to investigate the knowledge, attitude, and performance of workers of one of the waste recycling centers in Tehran metropolis regarding health aspects during the spread of Covid-19. Materials and Methods: This was a descriptive-analytical cross-sectional study and the study population was all workers working in one of the recycling centers of Tehran Municipality. The data collection tool was a researcher-made questionnaire with validity and reliability in four sections: demographic, knowledge, attitude, and practice of workers toward Covid-19 and solid waste. Results: Findings of the study showed that the highest number of workers employed were in the age group of 20 to 40 years (60 percent). The level of knowledge was about 80 percent and the lack of awareness was 20 percent. The level of workers' positive attitude, negative attitude, and lack of attitude were 79.71, 14.56, and 5.72 percent, respectively. The level of positive practice among workers was about 67.43 percent and the negative practice was 32.57 percent. Conclusion: The results clarified that in order to succeed in waste management programs in recycling centers, municipal waste managers should pay more attention to educational aspects, especially through the focus on programs and their development and adaptation for different age groups, especially 20 to 40 years. ABSTRACT

Patients with COVID-19 frequently experience neurological symptoms. Headaches and dizziness are common but non-specific symptoms. Both peripheral and central nervous systems can be impacted in severe stages. We focused on the neurological manifestations of COVID-19 patients in critical care. A cohort study evaluated the acute neurological manifestations in 204 patients admitted to intensive care units (ICU) tertiary Imam Khomeini hospital complex, Tehran, Iran. Patients with positive COVID-19 tests and severe clinical symptoms in both sexes, older than 16 years, were included in the study. Two groups of patients with positive or negative neurologic complications were compared by chi-square or Fisher exact test for categorical variables. The differences in continuous variables between the two groups were investigated using an independent sample t-test. The Kolmogorov-Simonov test was used to verify the normality assumption. A P less than 0.05 was considered statistically significant. The study included 204 individuals (130 males and 74 females) out of 270 ICU patients. Ninety (44.1%) patients were discharged, while 114 (55.9%) died. Overall, 17 (8.3%) patients had neurological complications, while 187 (91%) did not (P=0.005). The two groups did not have significantly different mean age (P=0.325) and sex (P=.793). The ventilation support was significantly different in the two groups (P=0.002). The death group had a higher incidence of loss of consciousness (P=0.003). COVID-19 causes neurological symptoms, especially during the inflammatory phase, and clinicians should be alert for neurological issues.
